Benefit Changes for 2017

This is the new benefits changes for 2017:

Preventive Immunization:  Flu, Pneumonia, Shingles and Tdap ( whooping cough, Tetanus, and Diphtheria)

Will be covered at a Tier 1 co-payment of $12 starting 8/1/2017.  In Pharmacy need to show Express Script card.

Medicare Advantage PPO Enhancement:

Only change is out of pocket Max use to be $3000. and will be $1500 in 2017

In network:

$490 per person
30% co-ins
Family $1395 per person
 Elimination of co- ins for DME Supplies. 

BC/BS TCN went up $15 for single and $25 for Family

Prescription Drug:
Mail Order:  Tier 1 no change
Tier 2 increase $5 - 2017 will be $85 for 90 days
Tier 3 increase $30- 2017 will be $230 for 90 days

Retail Pharmacy:
Tier 1 co-pay increase $2 to $14 for 30 days
Tier 2 $5 increase -$45 for 30 days
Tier 3 $15 increase to $115 for 30 days

You will be receiving highlights to all the changes in the mail starting in early August. Please review a lot of good material to read.

VERY IMPORTANT: AUDIT WILL GO OUT IN SEPTEMBER 12, 2016, YOU WILL NEED TO RETURN REQUESTED DOCUMENTS BY OCTOBER 31, 2016 TO AVOID ANY CANCELATION OF ANY DEPENDENTS.
